Please use this form to document when a client's details are not consistent across all Government records and needs to be updated before applying for a Director ID.

Does the client know what information needs to updated?*
E.g. Does the client know why their Director ID application has/will be been unsuccessful?
E.g. Name spelled differently in myGov or in Passport, incorrect date of brith etc.

Please include the incorrect and correct information clearly labelled, and what need to be changed.

Is a Statement of Corrections required?*
Please provide a copy of at least two out of the following documents: Passport, Birth Certificate, and/or Marriage Certificate.

If you have changed your name due to Marriage, please provide a Marriage Certificate as one of your identity documents.

Please do not upload a Drivers Licence, we require documentation that includes your place of birth.
